In case of company the following documents may be an evidence for closure of your ESI code No:
01. Proof of Sale of your machinery;
02. Full & final Settlement of workers along with evidence, such as acquitance of all the discharged employees towards full and final settlement;
03. Electricity Connection surrender;
04. Return of I.T. dept. till the date of closure;(for last 2 years)
05. Return of Sales Tax Department;
06. Telephone connection surrender;
07. Closing of Bank Account;
08. In case of company , intimation to Registrar of Companies;
09. Surrender of Trading Licence ,if any
10. Dissolution of rental deed, lease deed
11. Any other relevant documents for the same, if any

01. Affidavit from all the directors for any future liability
02. Return of I.T. dept. till the date of closure;(for last 2 years)
03. Return of Sales Tax Department;
04. Telephone connection surrender;
05. Closing of Bank Account;
06. In case of company , intimation to Registrar of Companies;
07. Surrender of Trading Licence ,if any
