Sub: Claim Form No. 31 – Instructions on submission of Declaration Form therewith.

Ref: Head Office circular of even number dated 04.08.2016 (at SI. No. 264 of office orders/circulars for 2016-17)

Sir/Madam,

Please refer to the above said subject.

2. It had been clarified vide circular of even number dated 04.08.2016 that the Declaration Form can be accepted in case of UAN based Claim Form No. 31 (UAN) without attestation of the employer. The same procedure may also be adopted for Claim Form No. 31 also.

3. In view of above, the attestation of an employer on the Declaration form would not be required for both Form No. 31 & Form No. 31(UAN). However, the Claim Form No. 31 shall continue to be attested by the employer.

4. This has approval of the Central P.F. Commissioner.
