6. License on Form 20-F is given for retail offer of medications determined in Sch. ‘X’. (Structure 19-C)

(ii) Form no 20-A  Rs. 500

(iii) Form no 21-A Rs. 500

(iv)  Form no 20-C Rs. 250

(v) Form no 21 – Rs. 500
