CBEC released Anti-Profiteering Application Form (APAF – 1) which is to be To be filed before Standing Committee/State level Screening Committee in terms of Rule 128 of CGST Rules, 2017. The form is of only one page and also includes instructions for filing.
Anti-Profiteering Application Form (APAF – 1) is with reference to a single Good/Service. In case of application for multiple Goods/Services, you need to make separate application for each Good/Service. Further After admitting the application, applicant may be asked to furnish any additional details as deemed necessary.
Filled application from must be send to State level Screening Committee (in case issues is of local nature) or to Standing Committee.

Applicant needs to furnish following general details about himself- Name, Code of Applicant, GST Registration No. and Proof of identity
In addition to above applicant need to give Name of the supplier and need to specify is he is a Manufacturer, Service Provider, Trader or anything else.
