2. Affidavit on Rs.20/- Stamp Paper [duly executed & signed by shareholder (including joint shareholder, if any)].

3. Original copy/certified copy of FIR /Police Complaint lodged against the lost/misplaced shares.

4. Certified copy of ID and address proof (self-attested).

5. Certified copy of PAN Card (self-attested).
