Subject: Extension of last date “for submission of Self Declaration form for claiming CPE Hours under Unstructured Learning Activities for the Calendar Year 2017″ – from 31st May, 2018 to 30^th June, 2018

This is for kind information of the members that the last date for submitting Self Declaration form for claiming CPE Hours under Unstructured Learning Activities for the Calendar Year 2017 – has been extended from 31^st May, 2018 to 30^th June, 2018.

The members who could not submit their self declaration form up to 31^st May, 2018 for the Calendar year 2017, are requested to kindly submit the same at the earliest and latest by 30^th June, 2018.
