  The “Form 16A applicable for non government ORganisations from A.Y. 2011-12 onwards” provided by you for download is not as per amended guidelines. You have provided the same old Form 16A.

  I also subscribe to the views of Ankit. As the format do not contain the address of CIT(TDS) which is mandatory as per format designed by Income Tax Department.
